Rescue Application
Please follow these instructions to ensure your document is submitted correctly:
1. Save the document to your computer;
2. Enter the requested information into the form fields;
3. In Adobe, select File --> Print, then select Adobe pdf as the choice of printer to print the completed document to pdf format, which will save the completed application to your computer; and
4. Return your completed application to us at rescue@westieclubhouston.com.
